A Brief Overview
Provides acute and rehabilitative care for patients with wound, ostomy, and continence care needs. Coordinates professional nursing and individualized, family centered care to patients with skin, wound, or ostomy issues to prevent and manage infections, reduce cost and simplify the workload of the bedside nurse using evidence based strategies for care.

Essential Functions

  • Conducts individualized wound/ostomy specific patient assessments based on consultations received from hospital staff. Assessment: Maintains expert knowledge in wound vac dressing changes and management of wound/ostomy care. Assists staff in recognizing Stage I and Suspected Deep Tissue Injuries Collects and documents data related to patients individualized care. Demonstrates wound/ostomy specific competency in assessment of patients by the ability to obtain and interpret information in terms of the patient needs, and understanding of the range of treatment needed by these patients. Assesses patient/family need for education and uses an individualized approach in delivery style. Assesses family expectations for, and involvement in the assessment and continuous care or treatment of the patient. Identifies nursing diagnoses, collaborative problems, and/or protocols for each patient, based on assessment. Assess and interprets patient data upon receipt of consultation. Reassessment: Performs reassessment based on team standards, CPGs/NDs, protocols and changes in patient condition. Communicates changes to appropriate persons (MD, Management, family) as indicated. Adjusts plan of care and treatment based on reassessment findings.
  • Implements and evaluates nursing actions in accordance with the individualized patient/family plan of care and patient condition/needs. Oversees that the nursing and interdisciplinary plan of care/goals includes skin, wound and ostomy care. Incorporates critical thinking into the delivery of clinical skills and in analyzing patient response to treatments. Demonstrates clinical competence in interventions for skin, wound and ostomy care. Provides and/or coordinates with other disciplines patient/family education according to identified care needs, considering assessment of patient/familys readiness to learn.
  • Documentation reflects accurate assessment/evaluation, modification, and patients response to therapy, changes in condition, and therapeutic interventions/procedures.
  • Develops wound/ostomy educational materials for health care team, patient and family: Develop and revise clinical pathways for wound care. Revise teaching sheets, policies, and CPGs related to skin integrity, wounds, wound vacs and ostomies. Provide educational materials for patients and staff in regards to common skin, wound and ostomy problems. Provide educational offerings/presentations for staff regarding skin, wound, and ostomy care. Revise the wound and ostomy formularies to assure the organization is providing excellence in wound and ostomy care in the most cost effective manner
  • Provides leadership in the monitoring of prevalence and incidence of pressure ulcers within the organization.
  • Develops, enters data, and compiles reports from skin, wound, ostomy data base.
  • Promotes pediatric research by implementing research findings into practice.
  • Accepts assignments to share/teach knowledge and provide clinical support.


Education Qualifications

  • Bachelor's Degree From an accredited college or university in nursing Required

Experience Qualifications

  • Minimum 3 years of experience in nursing required, preferably in pediatrics Required

Skills and Abilities

  • Must be able to perform a variety of duties, often changing from one task to another of a different nature, without loss of efficiency or composure, and be able to perform under stressful conditions when confronted with multiple requests, demonstrating prioritizing skills.
  • Strong interpersonal and communication skills with the ability to work effectively with multiple disciplines.
  • Legible handwriting skills.
  • Ability to be self-directed, with minimum supervision, and detail oriented.

Licenses and Certifications

  • RN - Registered Nurse - State Licensure and/or Compact State Licensure Required and
  • Current and valid Basic Life Support (BLS) through the American Heart Association Required and
  • PALS - Pediatric Advanced Life Support Required and
  • CWOCN - Certified Wound Ostomy Continence Nurse within 1 Year Required
