Nurse Practitioner (NP)

Titan Placement Group invites you to explore an opportunity in Ocala, FL providing primary care services in skilled nursing facilities (SNFs), assisted living facilities (ALFs), and private residences throughout Ocala/The Villages and Citrus County. This role is ideal for Nurse Practitioners dedicated to geriatric care, chronic disease management, and reducing hospitalizations through proactive and preventive medicine.

Salary and Benefits

• Salary: $100,000 annually
• Incentive Plan Available

• PTO: 12 days per year
• Paid Holidays: 6 per year
• Retirement: SIMPLE IRA
• Continuing Medical Education (CME): $1,000 annually

Responsibilities

• Provide on-site primary care services in SNFs, ALFs, and private homes
• Conduct comprehensive assessments and develop care plans tailored to each patient's needs
• Work closely with support staff, families, and facility teams to ensure optimal patient outcomes
• Conduct telehealth visits as needed
• Participate in Medicare s Merit-based Incentive Payment System (MIPS)
• Join weekly virtual provider meetings via Teams
• Maintain accurate documentation and billing in eClinical Works (EHR)
• Participate in on-call rotation for after-hours patient needs
• Meet production goals of:
  • 18 visits per day in ALFs
  • 20 visits per day in SNFs
  • 9 visits per day for home visits

Requirements

• Active Florida Nurse Practitioner (APRN) license
• DEA License
• At least one year of experience in geriatrics preferred, but not required
• Experience with eClinical Works (EHR) preferred
• Strong clinical skills, communication, and ability to work independently
