Internal Medicine Physician

Join a well-rounded team of 4 physicians (2.6 FTE), 5 Nurse Practitioners (4.27 FTE), 3 MA's, 1 RN, 1 Social Worker, and 1 lab assistant help to take care of our most medically complex community members through homes and skilled nursing facility visits. • BC/BE Geriatrics, Family Medicine or Internal Medicine Physician, Geriatrics Fellowship preferred
• Flexible 4-day or 5-day work week
• 8 - 10 patients per day
• Phone call - 2x per month and 1 weekend quarterly shared between Geriatrics and Internal Medicine teams
• Home-based care and coverage of 3 local skilled nursing facilities
• Work closely with Palliative Care and Hospice teams
• Epic Electronic Medical Record
