We areseeking a dedicated Psychiatric Mental Health Nurse Practitioner or PhysicianAssistant to join our organization, providing integrated psychiatric care forgeriatric patients. This role is ideal for a practitioner who thrives in acollaborative, multidisciplinary environment and is passionate about populationhealth and psychiatric-integrated primary care. You will focus on managingsub-acute and acute psychiatric care, ensuring seamless transitions back intocommunity-based care after a 3-6 month follow-up period.
Compensation & Benefits:
Base Salary: $170,000 - $190,000plus annual bonus.
Full benefits package includinghealth insurance, retirement plans, CME allowance, and PTO.
Role Overview:
Manage medication management forgeriatric patients within the PACE program.
Provide psychiatric care forpatients requiring short-term stabilization (3-6 months) before transitioningback into community support.
Collaborate with primary careproviders and multidisciplinary teams to ensure holistic patient care.
Work within a managed care modelto integrate behavioral health services with primary care.
Conduct psychiatric evaluationsand follow-up visits in outpatient clinic settings with some telemedicine.
Coordinate care plans with socialworkers, nurses, Psychiatrists, primary care providers and other healthcareprofessionals.
Focus on psychiatric management,this position does not involve therapy.
Contribute to improving populationhealth outcomes through proactive psychiatric interventions.
Requirements:
Board-certified Psychiatric MentalHealth Nurse Practitioner or Physician Assistant
2 years of experience asPsychiatric Provider
Experience in geriatricpsychiatry, population health, or integrated primary care preferred.
Strong ability to work within amultidisciplinary, team-based approach.
Experience in managed caresettings is a plus.
