Currently, we have two facilities needing coverage one is just northwest of downtown the other is just southeast of downtown. Both facilities are within 30 minutes of each other.
Role Overview:- Schedule: Have scheduled patients 4 day a week
- Provide patient care in a skilled nursing facility by conducting rounds in their short-term units
- Conduct documentation at home with provided iPad laptop within 48 hours of seeing patients
Key Responsibilities:- Perform assessments and therapy sessions
- Collaborate with facility staff to enhance patient care
- Ensure timely and accurate documentation
Requirements:- Hold a valid full license as either a licensed professional counselor or a licensed mental health counselor
- Experience in healthcare or long-term care is preferred but not required
Compensation:Expected compensation is estimated compensation (fee for service) of $50,000 to $70,000 with no cap productivity income potential.
